To register a child for soccer, please fill out this form. ONE SEPARATE FORM FOR EACH CHILD. Once completed and submitted you will receive a notification to your e-mail, please contact us if you do not receive the notification. This form requires payment by Credit Card. If you are paying by cheque go back to https://www.boundaryyouthsoccer.com/programs/house-u7-to-u16/ and choose Cheque. ---------------- ----------- Families applying for a Subsidy follow the same procedure but instead of a cheque you must send us the Subsidy Application form completed of which we will forward to Kidsport BC; once received we will place your child onto a team. Subsidy form is available on our website under the Kidsport BC link.

BYSA is not a drop in soccer program. A commitment is required upon registration both by the parent or guardian and the child. Commitment means participating as a team player, and showing up to all games and practices.

By submitting this form, I hereby voluntarily accept any and all risks for myself and any minor I register that may be associated with participating in the above mentioned program and associated activities and in consideration for being allowed to participate in the program, waive any rights I may have now or at any time hereafter have against Boundary Youth Soccer Association (BYSA), their respective officers and volunteers (referred to hereafter collectively as the Boundary Youth Soccer Association) and release the BYSA from any and all claims, actions, damages and losses whatsoever in relation to any loss of or injury to property and personal injury. Information collected here is solely for the use of the BYSA and will not be divulged to anyone else. As part of it's Web site the BYSA acquires photos of players in soccer situations. Pictures of BYSA activities may be posted on the website. The BYSA does not add names to the photos. The BYSA may post pictures on the website or by e-mail to the membership. All pictures become the property of the BYSA. The BYSA will not use the submitted photos for any other purpose than to share the photos fun and to propmote soccer for the association. The BYSA does not guarantee the posting of all pictures submitted nor the time they will remain posted. I attest that I have the legal right to accept the aforementioned risks associated with this program or any other BYSA sanctioned event, for any minor that I may register or bring in contact with in such circumstance.

Fair Play Policy
If children are to grow and develop in their sport or physical activity, an environment of positive communication and respect must exist. Parents should observe the following Code of Conduct with their child athletes. The following code is taken from a resource manual developed for community coaches by the Canadian Centre for Ethics in Sport (CCES). • I will remember that my child plays sport for his or her enjoyment, not for mine. • I will encourage my child to play by the rules and to resolve conflicts without resorting to hostility or violence. • I will teach my child that doing one's best is more important than winning, so that my child will never feel defeated by the outcome of a game/event. • I will make my child feel like a winner every time by offering praise for competing fairly and trying hard. • I will never ridicule or yell at my child for making a mistake or losing a competition. • I will remember that children learn best by example. I will applaud good players performances by both my child's team and their opponents. • I will not force my child to participate in sports. • I will never question the official's judgement or honesty in public. • I will support all efforts to remove verbal and physical abuse from children's sporting activities. • I will respect and show appreciation for the trained volunteer coaches who give their time to provide sport activities for my child, understanding that I have a responsibility to be a part of my child's development.

As a parent, spectator and supporter of kids sports I promise to not interfere with the players game by being a sidline coach, by questioning the coach or ref during games, by being overly loud or abusive in any way to any person at any event, I will support all players by encouraging fair play, accepting losses with as much grace as a win, by not demanding of players unrealistic goals and to accept their perception of events as important and real. I will show respect to all players' efforts and support equal time for all kids to play regardless of talent or "stature" and above all I will remember that this is a kids game for kids' to have fun.
