To register a child for soccer, please fill out this form. ONE SEPARATE FORM FOR EACH CHILD. Once completed and submitted you will receive a notification to your e-mail, please contact us if you do not receive the notification. This form requires payment by Credit Card. If you are paying by cheque go back to https://www.boundaryyouthsoccer.com/programs/house-u7-to-u16/ and choose Cheque. ---------------- ----------- Families applying for a Subsidy follow the same procedure but instead of a cheque you must send us the Subsidy Application form completed of which we will forward to Kidsport BC; once received we will place your child onto a team. BYSA is not a drop in soccer program. A commitment is required upon registration both by the parent or guardian and the child. Commitment means participating as a team player, and showing up to all games and practices.
Awareness of Risk
By submitting this form, I hereby voluntarily accept any and all risks for myself and any minor I register that may be associated with participating in the above mentioned program and associated activities and in consideration for being allowed to participate in the program, waive any rights I may have now or at any time hereafter have against Boundary Youth Soccer Association (BYSA), their respective officers and volunteers (referred to hereafter collectively as the Boundary Youth Soccer Association) and release the BYSA from any and all claims, actions, damages and losses whatsoever in relation to any loss of or injury to property and personal injury. Information collected here is solely for the use of the BYSA and will not be divulged to anyone else. As part of it's Web site the BYSA acquires photos of players in soccer situations. Pictures of BYSA activities may be posted on the website. The BYSA does not add names to the photos. The BYSA may post pictures on the website or by e-mail to the membership. All pictures become the property of the BYSA. The BYSA will not use the submitted photos for any other purpose than to share the photos fun and to propmote soccer for the association. The BYSA does not guarantee the posting of all pictures submitted nor the time they will remain posted. I attest that I have the legal right to accept the aforementioned risks associated with this program or any other BYSA sanctioned event, for any minor that I may register or bring in contact with in such circumstance.
Fair Play Policy
If children are to grow and develop in their sport or physical activity, an environment of positive communication and respect must exist. Parents should observe the following Code of Conduct with their child athletes. The following code is taken from a resource manual developed for community coaches by the Canadian Centre for Ethics in Sport (CCES). • I will remember that my child plays sport for his or her enjoyment, not for mine. • I will encourage my child to play by the rules and to resolve conflicts without resorting to hostility or violence. • I will teach my child that doing one's best is more important than winning, so that my child will never feel defeated by the outcome of a game/event. • I will make my child feel like a winner every time by offering praise for competing fairly and trying hard. • I will never ridicule or yell at my child for making a mistake or losing a competition. • I will remember that children learn best by example. I will applaud good players performances by both my child's team and their opponents. • I will not force my child to participate in sports. • I will never question the official's judgement or honesty in public. • I will support all efforts to remove verbal and physical abuse from children's sporting activities. • I will respect and show appreciation for the trained volunteer coaches who give their time to provide sport activities for my child, understanding that I have a responsibility to be a part of my child's development.
Parent Code of Conduct
As a parent, spectator and supporter of kids sports I promise to not interfere with the players game by being a sidline coach, by questioning the coach or ref during games, by being overly loud or abusive in any way to any person at any event, I will support all players by encouraging fair play, accepting losses with as much grace as a win, by not demanding of players unrealistic goals and to accept their perception of events as important and real. I will show respect to all players' efforts and support equal time for all kids to play regardless of talent or "stature" and above all I will remember that this is a kids game for kids' to have fun.
BYSA relies on its members volunteer efforts to make each season a success, without it we cannot function. There are lots of options to get involved; some tasks are larger, some smaller, the majority are short moments and usually quite fun. Many hands make for light work. Due to COVID-19 and the state it has thrown us all into BYSA is able to run this program without having to charge any fees, although in the interest of all we hope that players registering will commit to themselves and other players by attending all events.


Please review the next Paragraphs very carefully as they are integral to us being able open and operate. By Accepting them as required you will be agreeing to have understood them and that you are aware of the waiver you agreeing to for the player you are registering

Boundary Youth Soccer Association’s Return to Play Plan

In response to COVID-19 please follow these guidelines to protect yourself and others: Please distance yourself 2 meters between each other. Gatherings will consist of maximum 50 people per regular 11 v 11 full sized soccer field. Limit the number of spectators watching (one parent only), if possible, please drop off your child off no more than 15 minutes prior to activity and pick up promptly to prevent overlap from other groups. All participants will wash/use hand sanitizer before and after their session. Participants will not share any equipment during practice times. Participants will bring their own water and not share with others. Participants will not share any food with others. Participants will sanitize the equipment used throughout the practice with approved cleaning products provided by the association. Cones will be used to divide the distance between participants. Participants will notify BYSA if they experience any COVID-19 symptoms within the last 14 days. Participants will stay home if they are feeling sick and remain home for 14 days if experiencing COVID-19 symptoms. Participants acknowledge that there are at risk when participating in soccer activities, and that measures taken by the COVID-19 Response Plan and Return to Sport Protocols, will not eliminate all risks. Coaches, volunteers, and participants will verbally confirm that they are not experiencing and signs of COVID-19 each session. Coaches will keep a list of the participants who attended each session in the case that follow up is required. If First Aid is required a mask and gloves are required.


*If a participant is a minor and the agreement is signed by a parent or guardian Application - all athletes, coaches, members, volunteers, participants, and family members of participants while in attendance at club activities (“Participants”) All Participants of agree to abide by the following points when club participating in club activities under the COVID-19 Response plan and RTP Protocol: • I agree to symptom screening checks and will let my club know if I have experienced any of the symptoms in the last 14 days. • I agree to stay home if feeling sick and remain home for 14 days if experiencing COVID-19 symptoms. • I agree to sanitize my hands upon arrival and departure, with soap or sanitizer. • I agree to sanitize the equipment I use throughout my practice with approved cleaning products provided by the club (shared and personal equipment). • I agree to continue to follow social distancing protocols of staying at least 2m away from others. • I agree to not share any equipment during practice times. • I agree to abide by all my Association’s COVID-19 Policies and Guidelines. • I understand that if I do not abide by the policies/guidelines, that I may be asked to leave the club for up to 14 days to help protect myself and others around me. • I acknowledge that continued abuse of the policies and/or guidelines may result in suspension of my club membership temporarily. • I acknowledge that there are risks associated when participating in club activities, and that the measures taken by the club and participants, including those set out above and under the COVID-19 Response Plan and Return to Sport Protocols, will not entirely eliminate those risks.
